E Kavach Registration
- Visit the official E Kavach website.
- Click on the “Health Record Form” button to open the registration form.
- Fill in the required details on the form:
- Full name: Enter your full name as it appears on official documents.
- Provide your registered name in the health system.
- Enter your current contact number.
- Specify the hospital name and type where you work.
- Hospital NIN (National Identification Number): Enter the hospital’s National Identification Number.
- Provide the name of your state, territory, and Talluca.
